Qwo® is the first FDA-approved injectable medication for the permanent reduction of cellulite in the buttocks of adult women. This medication helps to break up cellulite at its root cause–the collagen bands thought to tug on the skin of the buttocks and thighs, resulting the the classic dimply appearance of cellulite. Qwo® also stimulates new collagen production and helps to redistribute fat cells in the treated areas, leading to smoother appearance of the treated skin.

What Causes Cellulite?
Contrary to what most people believe, cellulite is not fat; it is a combination of overly snug collagen bands, which help hold the skin onto the body, as well as age-related changes in the fat and skin around these bands. While we all have collagen bands, some of us are genetically predisposed to these bands becoming less stretchy and more stiff over time. As the inflexible collagen bands tug on the skin, they create the classic, bothersome dimples of cellulite which commonly affect the buttocks and thighs in women.
Cellulite often appears in people as young as in their twenties. As our skin thins and fat cells enlarge with age, pushing against the skin and associated collagen bands, the appearance of cellulite becomes more prominent. Muscle-strengthening exercises and a healthy diet, in general, do not effectively address these physiologic changes and do not significantly improve the appearance of cellulite. Liposuction, commonly used for body contouring and to get rid of bothersome deposits of fat, is also ineffective in the treatment of cellulite.
How Can Cellulite be Treated?
2020 saw lots of exciting news around Qwo®, the first FDA-approved injectable and permanent treatment for cellulite. Now available for treatment, Qwo® is an injectable medication that breaks up the collagen bands responsible for cellulite. It contains an enzyme called collagenase, which redistributes fat cells and stimulates the growth of new collagen in addition to lysing or destroying the collagen bands.
Collagenase has been successfully used in the past to treat other collagen-based conditions, such as Dupuytren’s contracture; this condition causes excessive scar tissue to form in the hand tendons, leading to a clenched appearance. Collagenase represents a significant paradigm shift in the approach to treating cellulite, as its role is to chemically subcise or cut away at the overly snug collagen bands responsible for the dimpling of cellulite.
Dr. Inessa Fishman uses Qwo® to treat patients over the course of 3 treatments, performing a series of small injections directly to the bothersome dimples of cellulite. She uses a small needle to deliver the Qwo® medication under the skin to the collagen bands that tug at the skin. Treatments are typically performed 3-4 weeks apart, and patients report about 10 weeks before noticing visible results.
Who is a Candidate for Qwo?
Dr. Inessa Fishman conducts a detailed history and physical exam as part of a consultation for cellulite or other aesthetic concerns. A thorough consultation helps us determine whether your goals are in line with the results Qwo® can provide. In general, candidates for Qwo® cellulite treatments should be in good health and not taking prescription blood-thinning medications. In order to be considered a candidate for Qwo® you must be a woman at least 18 years of age with moderate to severe cellulite in the buttock region that you would like to improve. You should not be treated with Qwo® if you have an allergy to any collagenase or the ingredients in Qwo® or if you have an active infection in the treatment area.

How Safe is Qwo?

What is the Recovery from Qwo Like?
There is no significant social downtime after a Qwo® treatment. Moderate bruising and mild to moderate tenderness at the injection sites are common, and will generally dissipate in a few days after treatment. As the treatment sites can be easily covered with clothing, our patients most commonly return to work and their social obligations immediately after Qwo® treatments. Because bruising is a common side-effect of injections, we recommend you complete your 3-injection visit Qwo® treatments, separated by 3 weeks, well before or after bikini season. We also recommend compression (in the form of a Spanx®-like garment or tight yoga pants) for a few days after treatment, as this helps decrease swelling and bruising.
