Brow Lift Surgery: Restoring Upper Facial Harmony and a Natural, Refreshed Look

The brow does more than frame the eyes as it shapes every first impression. When it sits in a well, supported, naturally elevated position, the upper face looks open, rested, and approachable. As we age, however, the brow gradually descends. For some patients, the change is subtle. For others, it’s a meaningful driver of eyelid heaviness, a chronically stern resting expression, or deep horizontal forehead lines that no amount of injectable treatment fully resolves.

For the right patient, brow lift surgery offers a refined, lasting correction, one that restores upper facial harmony without creating an overdone or unnatural result.

Below, Dra. Fishman walks through the core benefits of brow lift surgery, how to recognize whether you may be a candidate, and how to think about timing.

What Is Brow Lift Surgery?

A brow lift also called a forehead lift is a surgical procedure that elevates and repositions the brow while smoothing the upper forehead. The goal isn’t an exaggerated arch or a “surprised” look. It’s a restoration: returning the brow to a naturally refreshed position that reflects how you actually feel.

At Aviva Plastic Surgery & Aesthetics, we customize the brow lift technique to each patient’s anatomy after evaluation like:

  • Forehead dimensions and hairline position
  • Degree and pattern of brow descent
  • How much of the upper eyelid heaviness is driven by the brow versus the eyelid itself
  • Overall facial proportions and symmetry

Brow lift surgery may be performed alone or combined with upper blepharoplasty or facelift when a more comprehensive result is the goal.

Dr. Fishman performed a brow lift, resulting in excellent healing and an already brighter, more refreshed appearance.

Benefits of Brow Lift Surgery

For appropriately selected patients, the improvements are meaningful and, importantly, natural, looking.

A more rested, approachable appearance. When the brow descends, patients frequently report that others ask if they’re tired or upset, even when they feel completely fine. Elevating the brow restores openness to the upper face.

Reduction of upper eyelid hooding. In many patients, what looks like excess eyelid skin is actually driven at least in part by brow position. Correcting the brow can reduce lateral hooding and significantly improve the result when eyelid surgery is performed at the same time.

Softer forehead lines. Most patients don’t realize they’re doing it, but chronically elevated brows the body’s automatic compensation for heaviness deepen horizontal forehead lines over time. Repositioning the brow reduces that compensatory strain.

Improved upper facial balance. The brow, eyelids, and forehead function as a unit. Proper brow position brings the entire upper face into better harmony.

A durable, structural result. Neuromodulators like Botox can produce a mild, temporary brow elevation in select patients but they cannot correct true tissue descent. Surgery repositions the underlying structures for a lasting improvement.

Are You a Candidate for Brow Lift Surgery?

You may be a good candidate if you’ve noticed one or more of the following:

  • Brows that have gradually lowered over the years
  • Heavy or hooded upper eyelids
  • Outer brow drooping that creates a tired or stern look
  • Deep horizontal forehead lines from habitual brow raising
  • Asymmetry between the two brows
  • Limited or diminishing results from Botox or similar injectables

The most important step is a thorough evaluation. During consultation, Dr. Fishman performs a detailed upper facial analysis to determine whether your concerns stem primarily from brow descent, eyelid skin excess, muscle activity, or a combination because that distinction directly shapes the treatment plan and determines whether surgery will actually address what’s bothering you.

When Is the Right Time to Consider Brow Lift Surgery?

There’s no universal “right age.” Timing is personal as it depends on your anatomy, how your concerns are progressing, and what outcome you’re working toward.

If you’re noticing gradual brow descent. Early evaluation lets us determine whether subtle surgical repositioning now may provide more durable benefit than waiting until changes are more advanced.

If you’re planning upper eyelid surgery. In many patients, combining a brow lift with upper blepharoplasty produces the most balanced upper facial result. Addressing only the eyelids when brow position is also contributing can lead to an incomplete or shorter, lived outcome.

If injectables are no longer enough. Botox and similar treatments have a real ceiling. When true structural descent is present, surgery is typically the more appropriate and more efficient path.

If you want a longer, term solution. Patients who prefer to invest once in a lasting result rather than maintaining repeated temporary treatments often find brow lift surgery to be the more practical choice over time.

When your schedule allows for proper recovery. Planning surgery during a period when you can heal comfortably, without professional or social pressure, leads to a better overall experience.

Brow Lift Surgery at Aviva Plastic Surgery & Aesthetics

Dr. Fishman’s approach to brow lift surgery is defined by precision, restraint, and a deep respect for natural facial expression. She is a fellowship, trained facial plastic surgeon, and believes the most successful results come from careful evaluation understanding not just what has changed, but how those changes relate to each patient’s specific anatomy and aesthetic goals.

Dr. Fishman’s philosophy:

  • Natural brow positioning never artificially arched
  • Carefully planned, hidden incisions
  • Avoidance of the “operated” look
  • Preservation of each patient’s individual facial character
  • Thoughtful combination with complementary procedures when clinically appropriate

The goal is always refinement never obvious surgery.
