Depending on the patient’s preferences, medical history, and any additional procedures being performed, surgery may be carried out under intravenous sedation (“twilight anesthesia”) or general anesthesia. Have available for recovery:

  • Plain Tylenol (acetaminophen)
  • Plain Benadryl (diphenhydramine)
  • Prescribed medications
  • Hydrogen peroxide
  • Q-tips
  • Ice packs or bags of frozen peas
  • Extra pillows or a wedge pillow
  • Mild shampoo
  • Easy-to-prepare meals and snacks
  • Good books, favorite music playlists, and entertaining streaming options

Before Surgery

  • The evening before surgery, eat a healthy, protein-rich dinner and hydrate well.
  • Consider taking over-the-counter Melatonin or Benadryl if approved by your physician and helpful for sleep.
  • Do not eat or drink after the instructed cutoff time provided by your anesthesia team.
  • On the day of surgery, wear a button-front or zip-front shirt and slip-on shoes.
  • Please arrive with a clean face and hair. Avoid makeup, lotions, sunscreen, hairspray, gels, powders, or other hair products.

  • For the first 48 hours, spend most of your time resting, walking around the house periodically, and keeping your head elevated.
  • Sleep with your head elevated on 2–3 pillows or in a recliner for the first week.

  • Avoid strenuous exercise, heavy lifting, yoga inversions, and contact sports for 3 weeks or until cleared by Dr. Fishman.
  • You may drive once you are no longer taking prescription pain medication and feel comfortable turning your head.

Swelling and Bruising

  • Swelling of the forehead, scalp, upper eyelids, and around the eyes is common and expected.
  • Bruising may occur around the forehead and eyelids and typically improves over 2–3 weeks.
  • Swelling is often greatest during the first 2–4 days after surgery and gradually improves thereafter.
  • Temporary asymmetry is common during healing and usually reflects differences in swelling.
  • Some swelling may migrate downward into the eyelids and upper face before resolving.

Scalp Tightness, Numbness, and Sensation Changes

  • Tightness of the scalp and forehead is expected after surgery and gradually improves over several weeks.
  • Numbness of the scalp behind the incision is very common and may persist for several months.
  • Tingling, itching, “pins and needles,” or unusual skin sensations are normal signs of nerve recovery.
  • Most sensation improves gradually during the first several months after surgery, although some patients may experience small areas of persistent numbness.

Incision Care

  • You may shower and gently wash your hair approximately 24 hours after surgery unless instructed otherwise.
  • It is safe for clean water and mild shampoo to run over the incision.
  • Gently remove dried blood or crusting with hydrogen peroxide on a Q-tip if instructed by Dr. Fishman.
  • Apply any prescribed ointment as directed.
  • Avoid scratching, rubbing, or manipulating the incision.

Sutures and Staples

  • Sutures and/or staples are typically removed approximately 7–10 days after surgery.
  • Do not attempt to remove sutures, staples, scabs, or crusting yourself.

Hair and Hairline Changes

  • Temporary shedding of hair around the incision can occur and is usually temporary.
  • Hair growth near the incision may initially appear irregular as healing progresses.
  • Hair follicles along the incision are preserved whenever possible, and hair often grows through and around the scar as it matures.
  • Final scar maturation typically occurs over 6–12 months.

Clothing

  • Wear clothing that fastens in the front or back for the first week.
  • Avoid pulling shirts, sweaters, or turtlenecks over your head.
  • Avoid activities that place tension on the scalp or incision.

Sun Protection

  • Avoid direct sun exposure for at least 6 weeks.
  • Wear a hat when outdoors.
  • Once the incision has adequately healed, use sunscreen with SPF 30 or greater to help minimize scar discoloration.

Recovery Timeline

Every patient heals differently, but most patients:

  • Return to desk work and non-strenuous activities within several days to 1 week.
  • Have sutures and staples removed approximately 7–10 days after surgery.
  • Feel comfortable appearing in most social settings (“restaurant ready”) by approximately 3 weeks after surgery.
  • Prefer to allow approximately 8 weeks before weddings, graduations, professional photography sessions, reunions, or other important social events.

The hairline position is immediately lower after surgery, but swelling and early healing changes continue to improve over several weeks. Incisions continue to mature and soften over many months.

It is important to note that the first week is often the most dramatic-looking, not the most painful.

Hairline lowering patients frequently expect significant pain and are surprised by how manageable discomfort is. 

It is very common to have temporary swelling, forehead tightness, and scalp numbness after surgery.


Please Contact Dr. Fishman If:

  • Swelling suddenly increases on one side
  • Redness continues to worsen rather than improve
  • You develop significant bleeding
  • You have severe pain not relieved by medication
  • You have a fever greater than 100.4°F
  • You develop yellow or green drainage or a foul odor
  • You notice separation of the incision
  • You experience severe nausea, vomiting, or medication reactions
  • You have any questions or concerns

Medications, Supplements, and Alcohol

Please avoid aspirin, ibuprofen, naproxen, and other NSAIDs for 2 weeks before and after surgery unless specifically approved by your physician.

Avoid alcohol for 3 days before and 3 days after surgery.

Please discontinue supplements known to increase bleeding risk for approximately 3–4 weeks before surgery, including:

  • Fish oil / Omega-3 supplements
  • Vitamin E
  • Garlic supplements
  • Gingko biloba
  • Ginger supplements
  • Turmeric / Curcumin
  • Saw palmetto
  • Bromelain
  • Feverfew
  • Dong quai
  • Red clover
  • Reishi mushroom supplements

If you take prescription blood thinners, please discuss these medications with both the prescribing physician and Dr. Fishman before making any changes.


Smoking and Nicotine

Smoking, vaping, nicotine gum, nicotine pouches, nicotine patches, and all nicotine-containing products impair blood flow and wound healing.

Please avoid nicotine-containing products for at least 6 weeks before surgery and 6 weeks after surgery.

Nicotine use increases the risk of:

  • Poor wound healing
  • Infección

  • Skin loss
  • Need for additional surgery

For optimal healing, complete nicotine avoidance is strongly recommended.
