What You Need to Know About Eyelid Surgery

Who is a Candidate for Eyelid Surgery?

Blepharoplasty is an excellent option for patients who are looking for a way to refresh their appearance without resorting to more invasive surgery. The best candidates are men and women who have:

  • Droopy or sagging eyelids (upper, lower or both)
  • Excess fat leading to a baggy appearance of the eyelid
  • Upper eyelids that droop into the line of vision
  • Excessive wrinkles on either eyelid

When considering eyelid surgery, it’s important to consider the appearance of related features. The appearance of your eyelids is uniquely influenced by other facial features, particularly the forehead and eyebrows. As these areas begin to show signs of aging, they may pass these signs down to your eyelids – causing treated eyelids to droop again. In these instances, patients should consider combining their upcoming eyelid surgery with a brow or forehead lift.

I had my upper eyelids done. Dr. Fishman called me at home to see how I was doing. Doctors haven't done that in over 50 years.  I was out and feeling great within a week.  4 weeks after surgery I had an eye exam, when I mentioned, having the procedure, they were all amazed because they couldn't tell.  February 2018 I had my lower eyelids done and a chemical peel.  Again, same bedside manner, calling and checking on me with the end results being perfect.

Brow Lift vs. Eyelid Surgery

While often referenced as the same procedure, brow lift and eyelid surgery are two distinct surgical procedures. Unlike eyelid surgery, brow lifts are designed to lift the skin and tissues above the eyebrow and reduce the appearance of wrinkles and creases along one’s forehead. By lifting sagging or asymmetrical brows, this surgery can help rejuvenate a person’s appearance while helping him or her look less tired or angry, and more alert and approachable.

Who is a Brow Lift Candidates​?

As brow lifts focus primarily on rejuvenating the upper portion of the face, candidates for this procedure are healthy adults who show moderate to severe signs of aging. These include:

  • Deep vertical wrinkles between the eyebrows, often called glabellar lines or “the elevens”
  • Horizontal wrinkles across the forehead and/or across the bridge of the nose
  • Sagging tissue that droops on top of or in front of the upper eyelid

Much like eyelid surgery, the brow lift can only go so far in transforming a person’s overall appearance. Combining the procedure with eyelid surgery gives some patients the best of both procedures—a smooth, wrinkle-free forehead and an alert, refreshed appearance. In some instances, patients find that a more comprehensive treatment strategy is necessary to help them achieve their goals. What to Expect During Your Visit

Treatment begins with a one-on-one consultation with Dr. Fishman, during which you will discuss current concerns and long-term objectives, and explore all available options for reaching these ends.

Dr. Fishman’s restorative eyelid and brow lift treatments are customized for each individual patient.  For upper eyelid blepharoplasty, Dr. Fishman places a small incision within the natural crease of the eyelid, ensuring it can be easily concealed after healing. Excess skin and fat are then repositioned or removed, and the incision is closed with sutures. For lower lid surgery, incisions are placed right below the lower lash line or just inside the lower eyelid. Excess skin and fat tissue are then removed or repositioned, and the incision is sealed with sutures.

Recovering from blepharoplasty typically takes up to two weeks. During the first week, patients can expect some minor discomfort, swelling and bruising which generally dissipates with the consistent application of cool compresses. Patients may also experience eye redness, irritation and swelling.  These common symptoms generally resolve within the first few weeks after surgery. During this time, we recommend patients avoid wearing eye makeup and contact lenses.

Browlifts can be performed either endoscopically (via several tiny incisions within the scalp) or via an open approach, with longer incisions behind the hairline.  During the endoscopic approach, small incisions are made in the scalp, through which the instruments are then inserted. All brow-lifting adjustments are therefore made below the skin’s surface and with minimal scarring.  When open browlifts, the incisions are placed within the hair-bearing scalp of the temples or just in front of the frontal hairline.  The skin is then lifted, excess tissue removed, and the incisions closed with sutures or adhesive.

Similar to eyelid surgery, recovery after browlift is typically a two-week process, depending on the incision type and how your body heals. Patients can expect some discomfort and bruising near the incision site, which usually dissipates within a week after surgery. About two to three days after surgery, patients can wash their hair with a gentle soap. We recommend avoiding any strenuous activities or lying down completely for the first few weeks after brow lifting.
