What Causes Skin to Sag?
The aging process is inevitable. For many, fine lines begin silently developing around the late 20s to early 30s and gradually deepen into wrinkles with each passing year. The human face also loses volume due to changes in bony structure, the fatty tissues of the face, and decreased collagen and elastin production; these changes lead to sagging, lax tissues and skin within the face and neck.
Collagen and elastin are important compounds that give skin its ability to “bounce back” after trauma or damage. They also account for the well-supported, elastic skin and tissues we associate with youth. When we’re young, our body‘s production of abundant collagen and elastin gives skin its youthful glow and smooth, taut appearance. As we get older, however, collagen and elastin production slows; as a result, skin health and appearance begin to wane. Without the structure provided by collagen and with age-related volume loss, skin begins to crease across the face and previously sculpted facial features like cheekbones start to thin and sag. Combined, these symptoms can make a person look older; most patients in their 50s and older have facial aging concerns best addressed with a facelift and necklift.
Reverse Aging with a Face or Neck Lift
Face and neck lifts help patients regain a more youthful appearance by diminishing telltale signs of aging, such as jowls and lax, sagging skin. While these procedures share some similarities, there are key differences between the two which make them optimal for different patients.
In general, a facelift repositions into a more youthful, higher position the facial volume that has slipped down into the jawline and jowls. This treatment is performed by lifting and supporting the muscles of the face and the connective tissues that envelops these muscles; the connective tissue is called the SMAS. The volume which has slipped from the cheekbones into the jawline creates excess fulness in the lower face; this volume is lifted back up over the cheekbones and upper cheeks—eliminating jowls, excess tissue around the smile lines, and loose skin within the face. Facelifting incisions are placed within the natural creases between the ear and the face, thus camouflaging the ultimate scars.
In contrast, the neck lift corrects the appearance of a “turkey neck” – excess skin that sags and wrinkles around and in front of the neck, excess fat under the chin, and neck muscle separation which can lead to vertical platysmal muscle bands. The neck lift addresses excess under-the-chin and jawline fat, droopy skin , and muscle separation, with an individualized neck lift plan created for each patient. Incisions are hidden below the chin and behind the ears, making them almost impossible to find once healed.

Another advantage of combining face and neck lifts comes during recovery. As both take about the same amount of healing time, combining the procedures allows for one recovery and one anesthetic and operating room experience, thus saving the patient money. Patients go into the operating room and under anesthesia just once. They take just one block of time off and attend to the swelling and bruising with ice and rest just once. Patients save money as well because the surgeon schedules the operating room and her nurses and other medical professionals one time rather than for multiple separate procedures.
During the two weeks following surgery, patients will experience some degree of swelling and bruising. We recommend applying cold compresses to the treatment areas as well as taking oral medication to manage discomfort and swelling. We instruct surgical patients to wear a compressive garment or “chin strap” during the first week after the face and necklift procedure. While patients can begin walking the day of surgery, we recommend avoiding all other strenuous exercise and heavy lifting for 3-4 weeks after your surgery. As Dr. Fishman monitors your recovery, she’ll be able to give you more personalized direction throughout the healing process.
This depends on the complexity of the case and the need for associated procedures. Dr. Fishman usually completes neck lift surgeries in 2 hours, with more complex cases requiring 3-4 hours.
All incisions leave scars, and scars do not disappear (though they flatten and lose their initial redness with time). Dr. Fishman carefully hides her incisions for face and neck lifting surgery in the natural creases between the face and ear, in hair-bearing skin, and in the natural crease under the chin. These scars are difficult to notice once healed.
Most patients complain of a sore or a bruised sensation after neck surgery, and frequently report muscle tightness within the neck muscles. These symptoms resolve over several days – weeks after surgery. Strong oral pain medication is frequently employed for the first 1-4 days after surgery, and cold compresses are helpful to ease discomfort.
