How do you treat facial aging without surgery?

A number of facial concerns can be addressed without surgery, says Dr. Fishman. Nonsurgical face and neck contouring and tissue tightening; wrinkle-relaxing injections of Botox and Dysport; and volumizing dermal filler injections can make for beautiful facial rejuvenation without surgery.  Nonsurgical facial rejuvenation shines in restoring lost facial volume, and the most commonly treated areas cover the thinning temples, upper and lower eyelids, cheeks, lips, chin, jawlines, earlobes, and the chest and hands outside the face.

I went into the practice knowing that I was not ready for a surgical procedure for my face. Dr. Fishman respected my choices and did an amazing job going over my treatment options. We chose to do Botox and fillers, and I came out of the office looking like myself--but better. What is a Non-Surgical Facelift?

Non-surgical facelifts vary significantly between patients and are customized to each individual’s needs and wants. Today, we offer a variety of different dermal fillers, each designed to  address a different area on the face or issue. For instance, where Dr. Fishman turns to Voluma, Radiesse, and Restylane Lyft to contour and lift cheeks, she prefers Restylane to smooth undereye hollows and Juvederm to smooth tiny, superficial lines.   Because the culprit behind forehead and “frown” wrinkles are the frequently flexing muscles that lie underneath the skin, she uses Botox and Dysport to relax these muscles and smooth the overlying skin.

Some of the most common non-surgical procedures included in non-surgical facelifts include:

Injectable dermal fillers are gel-like substances injected into areas where diminished collagen production and increased skin laxity have caused skin to lose important elasticity. When injected, the formula helps add important volume to these areas while smoothing the overlying skin. This allows patients to achieve a more youthful and rejuvenated, smoother facial appearance.

Many dermal filler treatments contain hyaluronic acid (HA), a naturally occurring sugar substance that attracts water molecules, giving skin a full, glowy appearance.  Normally produced by our bodies as part of our skin and the fluid that lubricates our joints, hyaluronic acid does a great deal to make skin firm and smooth. As we age, however, our production of hyaluronic acid diminishes, contributing to skin laxity and loss of volume. Hyaluronic-acid-based injectables like Restylane and Juvederm add back volume to key spots on the face so that men and women can look younger. Dermal fillers’ results appear immediately to some extent, but because hyaluronic acid puts collagen production in motion, it takes several weeks for final results to appear.  The initial treatment improvements only get better with time.

The type of dermal filler that can be used as part of a larger non-surgical facelift depends on where the filler is being used. For instance, fillers made for the cheeks are thicker and firmer – allowing the formula to stay in place longer. Dermal fillers used to add volume to lips are typically thinner and softer, so as to simulate the look and feel of natural lip tissue. Not all dermal fillers are made the same – resulting in different treatment longevity and results.

Botox and Dysport are popular additions to non-surgical facelifts due to their ability to address one of the main causes of wrinkles – facial muscles. Over the years, muscles tighten and tense, creating characteristic fine lines across the face; just as a piece of paper forms a deeper crease with repeated folding, the skin becomes more creased with repeated muscle contraction and flexing. As collagen production decreases and the external damage increases, these lines deepen and become more noticeable.

Botox and Dysport use highly refined and purified forms of the botulinum neuromodulator to relax treated muscles.  Treating certain facial muscles with muscle-relaxing medications helps smooth skin, improve its texture, and erase lines.  When used as part of a non-surgical facelift, Botox or Dysport are typically used to address wrinkles that appear along the forehead, around the eyes, at the bridge of the nose and/or around the mouth, and smooth overly large chewing muscles leading to a too-square jaw.

Skin laxity is arguably one of the most noticeable signs of aging that traditionally has required surgery in order to treat. Luckily, innovations in cosmetic technologies have made radiofrequency (RF) skin tightening a viable option. At Aviva Plastic Surgery & Aesthetics, RF skin tightening is a popular addition to non-surgical facelifts.

During this procedure, radiofrequency energy is applied to skin and the underlying connective tissue using a special device that allows the energy to treat the tissues thoroughly without damaging them.  The radiofrequency energy helps stimulate the body’s natural collagen and elastin production– improving skin texture from the inside out and outside in.  Beyond the initial improvement, the body’s tissues continue to regenerate and tighte, leaving patients with more taut, healthier looking skin.

With minimal downtime associated with radiofrequency skin tightening, most patients experience some mild swelling and redness after their visit.  These treatment side effects subside within several days of treatment.

Who is a Candidate for a Non-Surgical Facelift?

A key advantage of the non-surgical facelift is that the procedure can be tailored to almost any healthy patient.
