Factors That Can Affect Healing After Labiaplasty: What Patients Should Know

Labiaplasty is a delicate, highly individualized procedure that can significantly improve comfort, confidence, and quality of life. When performed thoughtfully and followed by appropriate aftercare, most patients heal beautifully and are very pleased with their results.

Like all surgical procedures, labiaplasty has associated risks, and these are increased when certain medical or behavioral factors interfere with the body’s ability to recover. Understanding the potential labiaplasty healing problems and risk factors allows patients and our expert team to work together to ensure good healing and optimize outcomes.

Below are some of the most important factors that can affect healing after intimate wellness surgery.

1. Smoking and Nicotine Use

Smoking is one of the most significant risk factors for wound-healing complications in any surgical procedure and labiaplasty is no exception. Studies indicate that nicotine use can increase the risk of healing complications by up to 50%.

Nicotine causes blood vessels to constrict, drastically reducing oxygen delivery to healing tissues. This can lead to:

  • Delayed wound healing
  • Wound separation (dehiscence)
  • Increased risk of infection
  • Poor scar quality

Important Note: This risk applies not only to cigarettes but also to vaping, nicotine patches, gum, and other nicotine-containing products. At Aviva Plastic Surgery, patients are required to stop all nicotine use well before surgery and remain nicotine-free throughout the healing period to protect both their safety and their aesthetic results.  We recommend 6 weeks of no nicotine use before and after labiaplasty.

2. Obesity and Elevated BMI

Obesity can increase the risk of labiaplasty healing for several reasons. Higher BMI is often associated with reduced tissue oxygenation, increased inflammation, and higher mechanical stress on incisions.

In labiaplasty specifically, excess pressure, moisture, and friction in the vulvar region can slow healing and increase the risk of wound breakdown.

This does not mean labiaplasty is impossible for patients with a higher BMI. However, it does mean that careful surgical planning, strict adherence to postoperative instructions, and realistic expectations are essential for a safe recovery.

3. Not Following Pre- and Post-Operative Instructions

Poor compliance with postoperative care instructions is an unfortunately common cause of healing problems. To ensure a smooth recovery, patients must be mindful of the following:

Prolonged Sitting or Pressure on Incisions

Extended sitting especially on firm surfaces places direct pressure on healing tissues. This can increase swelling, compromise blood flow, and contribute to wound separation. Patients are instructed to limit sitting time, use appropriate cushioning (like a donut pillow), and rest in a horizontal position frequently during early recovery.

Excessive Activity Too Soon

Returning to high-impact exercise, cycling, horseback riding, tampon use, or sexual activity too early can traumatize incisions and disrupt healing tissues.

Poor Wound Care or Hygiene

Skipping prescribed cleansing routines or topical treatments can increase infection risk and delay recovery. Keeping the labiaplasty incisions clean and covered with an antibiotic ointment is paramount during the immediate post-surgery period.

Ignoring Restrictions on Clothing

Compression is helpful to minimize swelling immediately after surgery, and we recommend  this in the form of snug leggings or bike shorts worn with supportive underwear and a pad during the initial healing period.  Garments that can irritate fresh incisions (like thongs) should be avoided.  healing phase.

4. Medical Conditions That Affect Healing

Many medical conditions may impair the body’s natural ability to heal, including:

  • Diabetes or poor blood sugar control

  • Conditions requiring immunosuppressive medications

These factors do not automatically disqualify a patient from surgery, but they do require careful preoperative evaluation and individualized planning to prevent labiaplasty healing complications.

5. Infection or Excessive Swelling

Infection after labiaplasty is uncommon, but can absolutely prolong healing and impact treatment results.  Prompt communication with your surgeon about worsening pain, discharge, odor, or wound apperance changes is critical. Early intervention often prevents minor infections from becoming major complications.
