Dr. Fishman is an artist! My revision rhinoplasty was nothing like my initial surgery--I didn't bruise and only took Tylenol because I was told to do so by the Aviva team. It took me years to build up the courage to have my nose revised, and in retrospect, I'm happy for this time because it brought me to Dr. Fishman. I couldn't be happier with her skills, her artistic vision, and her thoughtful approach to me and my wants and needs. I can breathe! My nose looks amazing! I feel like this is the nose I was meant to have all along.

Revision rhinoplasty is nose surgery that helps correct or improve the results of a previous nose job. It can improve the look, shape, or function of the nose. Revision rhinoplasty often addresses the external nose anatomy (the term “rhinoplasty” means to change the shape of the nose) as well as the nose septum (the inside partition of the nose which splits it into 2 nostrils or airways).
Some people are unhappy with how their nose looks after their first (or subsequent revision) surgery. Others may have trouble breathing, or feel their nose doesn’t match the rest of their face. Revision surgery can help address these issues by restoring nose tip support, removing excess scar tissue, rebuilding nose structures previously excessively removed, straightening a deviated septum, and many other treatment options.
Revision or secondary (or tertiary, etc) surgery is more complex than primary nose surgery. Scar tissue, weakened cartilage, or missing support structures can make surgery more difficult. That’s why it’s important to choose a surgeon with special experience in revision rhinoplasty. Regular follow-up during revision rhinoplasty recovery is also important to a predictable and favorable treatment outcome. Â
Yes, that is our goal. Dr. Fishman uses careful planning and artistic skill to create elegant, natural-looking results. Your nose should fit your face and look like it was always meant to be there.
Yes. Many people come to us because their first surgery made breathing harder, or did not improve breathing problems present before initial rhinoplasty surgery. We focus not only on how your nose looks—but also how well it works. Nasal breathing is the most natural way to breathe, and it impacts our overall wellness, sleep, energy levels, ability to smell and taste the world around us. We focus our revision rhinoplasty techniques to support the nose in breathing while also addressing aesthetic concerns.
We use advanced techniques such as:
If we need rib cartilage, we usually take a small piece from your own rib through a tiny incision within the chest and placed within the fold under the breast. Most patients do very well with this, reporting several days of soreness which is manageable with anti-inflammatory medications. Your own or autologous rib cartilage tissue helps us build a strong, natural-looking nose. If you and Dr. Fishman decide not to use your own rib cartilage (due to patient preferences, or surgical contraindications noted by Dr. Fishman)
Healing takes time. While some people notice improvement immediately after surgery, it can take 12 to 18 months for all swelling to go down and for the final nose shape to appear. Factors like skin thickness and oiliness, age, and tendency towards forming excessive scar tissue also contribute to the healing time for revision rhinoplasty
