This 32-year-old woman came to our practice interested in revision otoplasty. She was bothered by the prominence of her right and left earlobes and the pointy outline of the right upper ear. She reported an initial bilateral/ both-sided otoplasty at age 8 years old, at a California practice. She “hated how overly plastered” the ears looked after surgery, and underwent bilateral otoplasty revision with another surgeon in Atlanta many years later; this procedure made the ears more prominent on both sides, but added earlobe asymmetry and prominence on the right. She underwent another revision focusing on earlobes only in 2024, but didn’t feel this made a notable difference in the earlobe appearance.
Our Facial Plastic Surgeon Dr. Inessa Fishman treated this young woman with revision otoplasty, or surgical ear-shaping, to contour the ears and bring the overly prominent lower portions of the ears closer to her head. This patent’s otoplasty surgery involved removing previously placed silicone grafting from under the ear cartilage; contouring the overly prominent earlobes with reshaping of the lowest-most part of the helical rim; and excising a small piece of excess cartilage at the right upper ear. Our patient is shown 1 week after her surgery, and has a beautiful treatment outcome, with which she is already thrilled; due to the highly visible nature of her job, our patient did not feel comfortable sharing photos of her face, but wanted to share the ear-shaping results she had been seeking for years. Her ears will continue to “depuff” and become less red as swelling and bruising resolves.
While children treated with otoplasty do best with general anesthesia (or being asleep in the operating room), most adults can have otoplasty surgery with the help of relaxing by-mouth medications, laughing gas, and injected numbing medications. This patient had her otoplasty surgery while awake and relaxed with by-mouth relaxing medications and laughing gas in the comfort of our luxurious clinic, and had a great and comfortable experience.
